We ask that you bring your insurance card to each visit and be prepared to pay your co-payment at the time of your visit; this is an insurance company requirement. Please remember that each child requires a separate co-payment. If you are unable to pay at the time of service there is a billing processing fee of $20.00 attached to your co-payment.

Please be aware that each insurance company has many different types of policies. Policies may differ as to their co-pay, deductible and co-insurance payment requirements. (Click on Financial FAQ for further explanation.) Call your insurance company for details on what type of billing to expect.

If you change your insurance provider, you must inform Mountainland Pediatrics BEFORE your visit. (Click FORMS AND POLICIES for easy ways to update your information.)
