Appointments

To make an appointment at either our Thornton or Louisville office, please call 303-430-0823, Monday through Friday, 8:00am to 5:00pm. Our offices are closed from noon to1:00pm for lunch hour.

If you cannot keep an appointment, please cancel as far in advance as possible so that we can schedule another child during that time.

If a child will be accompanied by anyone other than a parent or legal guardian, they must bring a signed Authorization for Child Treatment form with them to the appointment.

Please arrive at least 5 minutes before your scheduled appointment to allow time for check-in. If you are more than 10 minutes late for your appointment, your child may need to be seen by a different provider than the one with whom you were scheduled. If another provider is not available at that time, we will reschedule your appointment for another day.

Sick Child Visits

When your child is sick, we understand how important it is to you to have your child seen on the same day. Same-day appointments are available during business hours, including winter Saturday hours at the Thornton Office that have been reserved for sick children. Please call 303-430-0823 as early in the day as possible so that we can schedule a same-day appointment. We encourage you to call and schedule a same-day appointment rather than just bringing your child into the clinic, in order to minimize your time spent in the waiting room. When you arrive, please note that there is a special section of the waiting room for sick children.

If an emergency should arise, please call 911. Emergencies are considered to be those conditions that are life-threatening: loss of consciousness, severe bleeding, seizure, severe asthma attack, etc. If you have a minor medical emergency (lacerations, cuts, mild fractures, sprains) during office hours, we will do our best to see your child immediately or refer you to the appropriate facility.

Well Child Visits

We recommend that you schedule Well Child Visits at least 4 weeks in advance. Examinations are based on current recommendations provided by the American Academy of Pediatrics. Click here for the Well Child Examination Schedule. In order to give each child the individualized care that he or she deserves, we ask that you not schedule sibling appointments together. When you arrive, please note that there is a special section of the waiting room for well children.

If you are calling to schedule a Well Child Visit or consultation visit, you may find it more convenient to call from 10:00am until noon or after 3:00pm when phones are less busy. Our offices are closed from noon to 1:00pm for lunch hour.

New Patients

Please see the New Patient section of this website for information on introductory visits for new patient families and for records/forms information.

Consultation

For your convenience, a nurse is available for telephone consultations to answer any routine healthcare questions you may have. You may first want to check the Signs and Symptoms sections under Is Your Child Sick and What's Going Around on our website. Please call for a telephone consultation Monday through Friday, 8:00am to 5:00pm so that the medical assistant/nurse can access your child's chart during your conversation.
